All scheduling of office visits is done by our Reception Desk. All scheduling of surgeries is done by the assistants for each individual Surgeon.

Except in the case of emergencies, it is not our policy to see patients without an appointment. We understand that it is tempting to bring someone with you in hopes that they can be seen as well during your appointment time, but in fairness to those who have waited for an appointment, we are not able to see anyone without a scheduled appointment. We appreciate your understanding in this matter.

Also, because of our volume of bookings, we are only able to address one problem per office visit. You will need to schedule additional appointments on separate days if you have multiple concerns.

You should receive a reminder call to confirm your upcoming appointment. Please give the courtesy of a 24 hour notice for cancellations.

Please bring the following items with you to appointments:
  • A copy of your insurance card(s) or other health coverage information. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with your carrier if you are in doubt. Please confirm that your referral has been received.
  • X-rays or MRI’s not older than twelve or eighteen months (if available). Please note that we are able to view on line X-rays and MRI’s from Atlantic General Hospital, Progressive MRI, and Peninsula Imaging.
  • Co-Pays
  • A photo ID
  • A current list of medications that you are taking.
  • For Self Pay patients, a $300 deposit is required at the time of check-in. For any balance over $300, we are willing to set up a monthly payment plan for your convenience.
We accept most major insurance plans, as well as Worker's Compensation. Out of Network Benefits welcome.

We will be glad to bill you insurance plan for you. Please understand that you as the patient are ultimately responsible for any unpaid or uncovered charges incurred during your care with us. For patients whose injury is work related, you must provide the office with the name and billing address of the Workman’s Compensation Insurance Company, a Case Manager’s name and telephone number along with a Claim Number. Understand that we will be required to provide documentation to your Case Manager so that they are able to efficiently manage your care.

If in the future you need to request certain medical records from our office, please be aware that the request could take 10 days – two weeks before you receive the information. We make every effort to fulfill all medical records requests as quickly as possible. All requests must be accompanied with a Request for Medical Records Release signed by the patient or their legal representative.

Please contact our Medical Records Department if you have any further questions.
